Cytology - Fine Needle Aspiration

WSLH Department: Cytology
WSLH Test Code: 92701 FNA
Availability: Monday-Friday
Turn-around Time: 2 business days
Recommended Uses: Evaluation of sample for premalignant, malignant, inflammatory or infectious changes.
Contraindications: Hematoma may occur after needle aspiration at the site. Pneumothorax may results from some lung FNA. Delay in fixation, transport and processing may cause cells to degenerate.
Additional Tests Performed:
Patient Preparations: Dependent on clinician and specimen site.
Specimen Requirements: Prepared slides. Needles should be rinsed in a balanced salt solution or cytologic fixative and the rinse transported as per Body Fluids (see collection instructions). Patient's full legal name must be written on the slide/container.
Specimen Handling & Transport: Transport ASAP or within 12 hours. Insert glass slide in plastic slide holder. Wrap container with absorbent paper and put in a biohazard bag. Put the biohazard bag and plastic slide holder into the styrofoam mailer. Place completed requisition form in the styrofoam mailer. Label the styrofoam mailer with the following: Your laboratory/clinic's address, WSLH Cytology Department address, Diagnostic specimen label (orange), Biohazard sticker. Transport via courier or shipping carrier with a tracking option strongly preferred.
Collection Kit/Container: FNA Kit and Mailers
Collection Instructions: Label slides with patient's full legal name, DOB, DOS and specimen site. Place one drop of material onto slide and smear with another. Immediately fix one slide in 95% ethanol or use spray fixative. Allow the other slide to air dry. Write "A.D." on the slide allowed to dry. Remainder of specimen collected is rinsed in balanced salt solution or container of cytologic fixative, such as ThinPrep PreservCyt.
Unacceptable Conditions: Slides broken beyond repair. Specimen that leaks out of the vial may not be processible.
Requisition Form: WSLH Cytology Form 141-NGYN
Required Information: Laboratory regulations require the following minimum information to be provided on the requisition form for a specimen to be accepted for testing: Patient name or unique identifier; date and time of collection, patient date of birth and sex, specimen type/site of collection, test request(s), clinician name and UPIN, and address for reporting results. Please be certain that name/identifier on the form matches that on the specimen label.
Results Include: Negative for malignant cells, atypical cells present; suspicious for malignancy; positive for neoplasm; positive for malignancy; and unsatisfactory specimen. Additional descriptive terminology appropriate to the site and diagnosis may be used. Results may also include inflammatory, bacterial, and viral findings.
Limitations: Findings must be correlated with history and other test results. Poorly fixed or air-dried samples impair evaluation. This laboratory can provide educational materials and demonstrations for interested clinicians. As with any laboratory test, both false negative and false positive results may occur.
Additional Tests Recommended:
Additional Comments: (1) Additional fine needle aspirates and/or biopsies may be requested if initial results are inconclusive. (2) Immunohistochemistry tests may be ordered at the discretion of the pathologist to further confirm or rule out differential diagnoses.
Methodology: Cytologic evaluation of fine needle aspiration from a variety of body sites.
Includes: Needle aspiration specimens of breast, bone, kidney, liver, lung, lymph node, pancreas, prostate, salivary gland, synovium, thyroid, and any mass or neoplasm that is found via palpation or radiologic guidance
