| Required Information: | Vaccine history and susceptibility results Laboratory regulations require the following minimum information to be provided on the requisition form for a specimen to be accepted for testing: Patient name or unique identifier, date and time of collection, patient date of birth and sex, specimen type/site of collection, test request(s), clinician name and UPIN, and address for reporting results. Please be certain that name/identifier on the form matches that on the specimen label which is required to have a minimum of two identifiers. |

| Courier Information: | Specimens submitted for public health surveillance are eligible for fee-exempt courier service. Contact Purple Mountain Dispatch at 800-990-9668 for specimen pick up (in Wisconsin) or use the VPD FedEx number if sending from out of state. |

| Limitations: | Cultures must be viable for identification. Mixed cultures may significantly lengthen turnaround time. It may not always be possible to discriminate between multiple serotypes within a serogroup due to a high degree of genetic similarities or due to the existence of DNA polymorphisms among single serotypes. Therefore, a serogroup will be assigned for isolates where a serotype cannot be identified. Isolates that are not able to be serotyped may be sent to a reference lab for further identification at no additional charge. |
