| Specimen Requirements: | Dacron-tipped lesion swab with plastic shaft, in viral transport medium |
| Collection Instructions: | Vigorously swab at base of a new lesion using a Dacron swab, and immediately place swab in vial of Viral Transport Medium (VTM). See detailed instructions listed under Collection Kit 18, Viral Specimens. |

Please be certain that name/identifier on the form matches that on the specimen label which is required to have a minimum of two identifiers. |

| Specimen Handling: | Store specimens at 2-8°C and transport with a frozen cold pack. Specimens must arrive within 15 days of collection. |
