| Recommended Uses: | Identification of bacteria of public health significance. These include bacteria exhibiting unusual resistance, bacteria linked to an outbreak or cluster of illnesses, bacteria isolated from a sterile body site or bacteria under surveillance by WSLH and/or the Wisconsin Division of Public Health. |
| Pre-approval: | Not required for surveillance requests. Pre-approval is required for other submissions. Please contact WSLH Customer Service (800-862-1013) or Wisconsin Division of Public Health (608-267-9003) to inquire about whether an isolate is acceptable for submission. |

Antimicrobial susceptibility, serotyping and/or genotyping may be performed for public health investigation purposes. No additional charges will be incurred. |

| Specimen Requirements: | Pure culture on agar slant. Agar plates not recommended for shipping. |

| Required Information: | Please include all pertinent clinical information and exposure history. Laboratory regulations require the following minimum information to be provided on the requisition form for a specimen to be accepted for testing: Patient name or unique identifier, date and time of collection, patient date of birth and sex, specimen type/site of collection, test request(s), clinician name and UPIN, and address for reporting results. Please be certain that name/identifier on the form matches that on the specimen label which must have at least two patient identifiers. |
| Rejection Criteria: | Do not submit mixed or non-viable cultures; do NOT freeze. |
| Specimen Handling: | Ambient temperature using appropriate infectious substance shipping container. |

| Courier Information: | Fee-exempt courier is available for surveillance specimens and those with pre-approval from DHS. Contact Purple Mountain Dispatch at 800-990-9668 for specimen pick up. |
