Required Information:

Mycobacterium species identification. 

Laboratory regulations require the following minimum information to be provided on the requisition form for a specimen to be accepted for testing: Patient name or unique identifier, date and time of collection, patient date of birth and sex, specimen type/site of collection, test request(s), clinician name and UPIN, and address for reporting results. Please be certain that name/identifier on the form matches that on the specimen label which is required to have a minimum of two identifiers.

Cultures may be transported to the laboratory at ambient temperatures. Avoid temperature extremes during transport.

Specimens submitted at the request of a public health department are eligible for fee-exempt courier. Contact Purple Mountain Dispatch at 800-990-9668 for specimen pickup.

NOTE: This test cannot be ordered until an organism has been identified in a culture. Submitters requesting this testing on the same requisition form as specimens received for Mycobacteria (AFB) Smear and Culture (MM00250) or Mycobacteria Identification (MM00253) will be contacted by WSLH Customer Service and notified that they will need to submit an additional requisition for this testing after an organism has been identified. Unless specific drug(s) susceptibilities are requested, WSLH will automatically send isolates for the appropriate drug panel for the organism identified.
