| Required Information: | Please include all pertinent clinical information, lab rule-out testing results, and exposure history. Laboratory regulations require the following minimum information to be provided on the requisition form for a specimen to be accepted for testing: Patient name or unique identifier, date and time of collection, patient date of birth and sex, specimen type/site of collection, test request(s), clinician name and UPIN, and address for reporting results. Please be certain that name/identifier on the form matches that on the specimen label which is required to have a minimum of two identifiers. |

| Specimen Handling: | Store specimens (other than blood culture bottles) in a refrigerator (2-8°C) and ship with frozen cold packs. Ship blood culture bottles at ambient temperatures. It is recommended that specimens are received in the laboratory within 24-48 hours of collection. |

Enhanced precautions should be taken when handling suspect specimens or isolates. |
